This is an annotated POLST (Physicians Orders for Life Sustaining Treatment) Form, which is extremely useful in surrogate-less patient situations (unrepresented by definition). This form is composed of check box answers to questions such as 'Would you like CPR performed' etc. In the case of unrepresented patients, many of whom are critically ill and require LST (life sustaining treatment), this form is extremely valuable to physicians.
